Physical Examination

Athletes must have a current sport qualifying physical exam on file in the Activities Office. Physicals are good for 12 months from the exact date of the last physical.

Once you have your physical completed by a doctor, you should scan it and submit the form to your online athletic account. The most important page is the Medical History page which must be marked medically eligible for all sports without restriction.
